So I know that this would seem to indicate an addon conflict, but my problem is that I have no way to confirm it because this bug only occurs infrequently. I finally discover that if I started disabling addons in bunches of five, restarting, and then another five, the the ones farther down on the list that were not disabled were eventually restored, at which point I was able to re-enable the disabled addons with no problem. The only icons you consistently see are custom buttons and some of the default Firefox icons like "Show Your History", "Shoe Sidebar", etc. You see a few "random" addons displayed properly in these images, but in later occurrences, no addon icons displayed properly.
